How Do I?

Report an infectious disease or suspect infectious disease?:

  • Reporting an infectious disease or suspect infectious disease?

For women, children and young families:

  • Get a free or low cost immunization
  • Get help with breastfeeding
  • Register for Childbirth or Breastfeeding education classes
  • Get a nurse visit after my baby is born
  • Qualify for WIC (Women, Infants and Children)
  • Make sure my child’s development is on track
  • Get education on car seats or get my car seat checked?

For the elderly and the disabled individuals that may need services:

  • Request Hospice or Palliative Care Services
  • Request an assessment for in-home services or to move to a facility
  • Receive information on community resources to help me remain independent and at home
  • Find a local home care agency
  • Request mental health services
  • Get an emergency call button/pendant

For licenses, plan reviews, and complaints: 

  • Get a license
  • Report an unsanitary condition

For locating fresh produce:
