Horizon Public Health Community Health Assessment (CHA)

Every five years, Horizon Public Health conducts a Community Health Assessment (CHA). The purpose of this assessment is to learn about the health status of the people living in Douglas, Grant, Pope, Stevens, and Traverse counties and identify areas for improvement. The CHA is used to develop a shared Community Health Improvement Plan (CHIP), which outlines actions to address priority health issues.

Horizon Public Health Community Health Improvement Plan (CHIP)

The Horizon Public Health Community Health Improvement Plan (CHIP) is a long-term plan that identifies health priorities, goals, objectives, and action steps aimed at improving the health of Horizon Community residents. The CHIP is used to set priorities, coordinate resources, develop policies to target efforts that protect, and promote health.

Horizon Public Health Strategic Plan

The Horizon Public Health Strategic Plan will serve as a guide as to how the organization can function better to serve its population through two identified priority areas. Each focus area and associated goal creates a broad statement of deliberate intent to elevate the health department’s performance and ultimately to positively impact the health of the residents of the five counties served by Horizon Public Health.

The purpose of this profile is to gain a better understanding of what the overdose epidemic looks like in the counties that Horizon Public Health serves and to make data-driven prevention decisions to improve health. This data is created and maintained by Minnesota Public Health.
