Family Home Visiting
Our nurses love to come out to visit parents and their new babies. We find it’s a great way to follow up on breastfeeding questions and other infant care questions that arise. We also provide home visits to families with questions about pregnancy, their baby or child’s development, home safety and injury prevention and nutrition. Horizon Public Health offers these programs:
- Supporting Hands Nurse-Family Partnership, a program for first-time mothers
This is a voluntary program for first-time moms that is offered at no cost thanks to funding support from a variety of other resources. Mothers will have a personal nurse visiting them at home, who can offer education and support during pregnancy and until your baby reaches two years of age. - Growing Great Kids
This is a program focused on prenatal and postpartum health, infant/child development and attachment, and life skills to build a strong family support system. Thanks to funding support from a variety of other resources, this program is available at no cost to you. At your convenience, a personal nurse will visit with you and your child at home (up to age 3 years of age) to provide education along with activities and materials to make parenting fun and meaningful.
