Case Management

Case managers assist persons that are on a home and community based waiver program in developing a person-centered and goal directed plan of care that meets an individual’s needs. The case manager will work with the person in the development, planning, implementation and evaluation of this plan. In addition, the case manager will authorize payment for the services needed to implement the plan.

The case manager will also provide chronic disease management, assist the person with needed resources, and make appropriate referrals. The case manager will visit you regularly to review the plan of care and make changes to services as needed.
